By signing the agreement, the resident acknowledges and accepts the house rules, policies, and guidelines of Lifeline Community Outreach Center, Inc. They waive their right to written notice and eviction proceedings and agree to leave peacefully if asked to do so.
The resident is solely responsible for their physical, emotional, and personal well-being and for the self-administration of their personal legally prescribed non-narcotic medications as directed by a physician.
Lifeline Community Outreach Center, Inc. is not liable for any physical or mental injury, including death, sustained by the resident or any guest they may have on the premises, nor for any loss or damage to personal property.
